Contains a supplemental listing of all physician Unique Physician Identification Number (UPIN) records added to the Physician Registry from July 2003 to July 2005. Enables the health care community to place the UPIN of the referring or ordering physician on the Medicare and other claim forms. Arrayed in State order&#x3b; physicians within a State are arrayed alphabetically by physician name.
