Student Interview: A student's opportunity to contribute to his or her evaluation and annual review.

I have developed this written interview, which I began using with some of my students over the last few years. In doing so, I was able to better assess a student's abilities and perceptions of being able to care for himself or herself and support classroom and/or vocational skills. It prevents that 'oh no' moment when something is revealed in a CSE meeting that you should know but don't surfaces, as much as possible. I give permission for the user of this book to copy the interview forms for use with their own students.
