The book moves in three movements. The First Witness documents what it looks like when systems designed to sustain life fail to address the lives inside them: a severely autistic son who arrived before the architecture of hope and despair; a ninety-seven-year-old woman who has asked to die and cannot; a diabetic man being taken apart piece by piece; a family standing outside a ventilator door. The central distinction - between biological life, which systems maintain, and biographical life, which they cannot see - runs through every chapter.
