As the long-term care demographics continue to shift and diversify, the greater the need to understand people as individuals. Achieving such understanding presents new challenges and considerations in assessment and care planning. While long-term care facilities once cared almost exclusively for the frail elderly, many of whom had also been diagnosed with dementia, today's populations are far more varied in age, disability, and psychosocial circumstances.
